In order to create a UMT's card, please click on UMT Card Application for register form on the website of UMT.

Step 1: Please read conditions and agree with issuing card 's standard of UMT Card Application, please choose Agree. If you do not agree, please choose Not agree.

Step 2: After that, please select the card type that you would like and click Next

Step 3: Fill in Registration form, fields * are required to be filled. After finishing, click on register.

Step 4: When your registration has been approved, please keep information that UMT inform confidentially including Card number, registered phone number to use for UMT's services.
